When you arrive at the office, we will ask for your questionnaire, to provide your insurance information, and take a comprehensive medical history.  In addition to your five year weight history, please bring a list of any medications you are taking.
 Our nurse will take your vital signs, calculate your Body Mass Index (BMI), and see your bariatric surgeon.  Dr. Bell or Dr. Johnson. They will go over your questionnaire with you, your medical history, and answer all of your questions so that you can make an informed decision.
